Creating a form for data entry

Hi,

1) I want to create a form for reporting the incident data. The form should be marked OPEN when filled. When filled, it will send notification to manager and after review and appropriate actions taken, manager will mark it CLOSED. Is there a way to achieve this OPEN and CLOSED logic?

2) Once filled, manager should be able to print the form as it is filled by the user to review it. We will be doing aggregate analysis only at the end of quarter. Is it possible to print it as a filled form by the manager just as a individual response instead of collective?
