I no longer use actions to save file.  You should also look at menu File>Automate>Batch...  Photoshop's batch feature is used to batch actions. Photoshop's batch dialog has many options one can override Save As steps recorded into actions. However I do not think more then one output paths can be accomplished. Photoshop scripting can easily support many output file paths.

 

As I wrote the Image Processor Pro plug-in script could be use to do what you want to do. The script is a very powerful Batch Image Processor.   The Image Processor Pro script is a Photoshop Plug-in with Action support programmed in.   To create the action you want just record a single step action that records menu File>Automate>Image Processor Pro...  What ever settings you use in the Image Processor Pro dialog while recording the Image Processor Pro step will be recorded into the step and passed to the image processor pro script when the action is played.  No dialog will be displayed when the action is played unless you turn on the step dialog in the action step.  If you do that the dialog will be displayed withe the recorded setting filled in.  You can change ant you wish to and the click Run in the dialog,  If the step's dialog is not turn on the recorded setting will be used by the script.
