Would you please confirm that the question you have about the LiveCycle form is with regard to a Shared Review?
If it is, would you please reply with the exact version(s) of Acrobat and Reader you are using.
Adobe is investigating a similar inquiry with regard to custom stamps and Shared Reviews in the most recent updates, 10.1.10 and 11.0.7.
I am wondering if that issue also covers images in form fields.
